Who qualifies?

May qualify if you are:
- Enrolled in Traditional Medicare
- Living with any type of dementia at any stage
- Living at home, not in a skilled nursing facility
Not eligible if you are:
- Enrolled in a Medicare Advantage Plan
- Receiving hospice care
- Enrolled in a PACE Program
