Scope: – All patients that meet eligibility and criteria
Nature: – Identify patients with cognitive impairment indicators
Eligible: – Medicare / Medicare Advantage Beneficiaries
Criteria: – High Risk
Age: – 60 years+
- Previous indicators
- Interaction with physician or family/friends
- Medical conditions
- Medications
- Loss of memory
- Problems with Language, Thinking, Judgement, Problem-solving difficulty
- Depression, Anxiety, Head Trauma
Cognitive Service Flow
- Annual Wellness Visit to include Cognitive Screening
- Office Visit to include Cognitive Screenings
- Preventative Health Service lines to include Cognitive Screenings

- Approved Relative Value Scale Screening
- GPCOG – Cognitive Screening
- Format: 9 Questions, Scale 0-9
- Scoring: 9 – No cognitive impairment (cognitively intact)
- Indicators: Score 5-8
- Interpretation: Some cognitive impairment; further information needed. Practitioner conducts an information interview.
- Score 4 or less: High potential for cognitive impairment; Assessment is indicated.
- Intervention: Referral for Cognitive Assessment via Licensed Mental Health Professional, Cognitive testing
- Interpretation, Management, Consultation, Follow-up, Monitoring via Preventative Health Service (AWV – CCM – PCM – RTM – IBH)
- Management: Office Visits, Testing, Updated assessments and plans, Medication therapy, Memory Loss Services, Community resources, Support Groups
- Interpretation, Management, Consultation, Follow-up, Monitoring via Preventative Health Service (AWV – CCM – PCM – RTM – IBH)
